Can I request my information?
As a client you can request access to, or copies of your health record(s), this includes scans or personal data we hold as part of your records.
The right of access, commonly referred to as ‘Subject Access’, gives individuals the right to obtain a copy of their personal data as well as other supplementary information.
About SAR – Subject access request
You have the right to know whether any information is held about you and a right to a copy of that information. MSI Reproductive Choices UK will only release that information if we are certain of your identity. We will not give you any information which identifies someone else unless that person agrees. Failure to provide sufficient identification and evidence can cause delays to your request or refusal where none is provided.
Security measures are set up at the point of creating a medical record. In order for us to confirm the identity of the client, it will be required to pass security. Please make sure this is available prior to submitting a SAR; if you are unsure or have forgotten, please call 0345 300 8090. If you do not include in your initial request, you will be required to provide it later in the process.
What you need to make your subject access request
Two copies of identification are required when submitting a request for your personal information. We will accept a passport, driving licence, medical card etc.
These documents must clearly show the name, current postal address, date of birth and signature of the person whose information is being requested.
If you are applying on behalf of a client, please submit one item of proof of the client’s identity and one item of proof of the requestor’s identity.
