Can I request my information?
As a client you can request access to, or copies of your health record(s), this includes scans or personal data we hold as part of your records.
About SARs – Subject access requests
The right of access, commonly referred to as ‘Subject Access’, gives individuals the right to obtain a copy of their personal data as well as other supplementary information.
What to expect
A SAR only entitles you to copies of your own personal information, not information about anyone else or copies of our policies and procedures.
By law, we have one calendar month to respond to a SAR once we have verified your identity, and we will only release your information once we have done this.
If you need proof of attendance for your place of work or education, you can request a discharge summary which summarises the care that you received at MSI. Please note that discharge summaries cannot be used as sick notes.

What documentation you will need
Two copies of identification are required when submitting a request for your personal information; proof of your identity, and proof of your address.
These documents must clearly show the name, current postal address, date of birth and signature of the person whose information is being requested.
If you are applying on behalf of a client, please submit one item of proof of the client’s identity and one item of proof of the requestor’s identity.
Additionally, security measures are set up at the point of creating a medical record which are required to identify our clients and pass security. Please make sure your security information is available when submitting a SAR; if you are unsure or have forgotten, please call us 0345 300 8090.

Once received, if we still need to verify your identity we will respond asking for further evidence before processing your request; once your identity is confirmed we will respond within one calendar month. Please allow this time to elapse before chasing any requests.
